In order to access your 2011 Schedule K-1 form you will need your assigned American Stock Transfer Investor ID number and the last four digits of the Social Security or Tax ID number located on your K-1 form. If you seek multiple K-1 forms, you will need to enter all Investor ID numbers filed under each Social Security or Tax ID number.
